You can submit an application for a CHRS through the Court. Fees are $95 per search.

In order for the Court to complete a CHRS, you must provide the full name of the individual, including middle initial, and the full date of birth. Partial names or dates of birth are not accepted.

Applications may be filed in person weekdays, from 9:30 AM to 4:30 PM, at:

Applications that are hand delivered by 11:30 AM will be completed by 4:30 PM the next business day. 

Fees are payable by check or money order made out to NYS Office of Court Administration.
