You can get medical records from a hospital, a doctor, or from the Department of Health's Bureau of STD.

To request a copy of a medical record from a hospital, call or write to the hospital holding the record. You must speak to the Medical Records Department and request a release of medical information authorization form from the hospital.

If the hospital does not respond to this request within a timely manner, you can file a complaint with the NYS Department of Health, Hospital Complaint Department.

You can also make a complaint or get assistance with getting records from hospitals that have gone out of business.

You can request a copy of your medical record from the Department of Health's Bureau of STD. You will have to provide information to complete an Authorization for Release of Health Information form.

You can also visit the NYC Department of Health's Chelsea Van, located near Chelsea Clinic at 9th Avenue and 28th street, to complete an Authorization for Release of Health Information form and give it to staff on site.
