By Mail

Mail a "Request for Copy of Aided/Accident Record" (Form PD301-161) to the Traffic Safety Officer of the precinct where the accident occurred. The form is available at any precinct.

Include a photocopy of a valid form of picture identification (for example, a driver's license) and a self-addressed stamped envelope.

In Person

Go to the precinct where the accident occurred with a valid form of picture identification (for example, a driver's license or passport).

You can request an accident report from the State if the accident occurred over 30 days ago. To request a report, you must complete and mail form MV-198C (Request for Copy of Accident Report) to the State.
